To learn more about the “Montefiore Difference” – who we are at Montefiore and all that we have to offer our associates, please click here.   Job Summary Interview clients, review records, and confer with other professionals to document individuals' mental and physical condition and to determine their suitability for participation in a specific program track including Wellness, Federal Disability, employment with accommodations, or employment without limitations. Conduct the Social Security Administration Sequential Evaluation as part of the assessment process. Initiate CASAC and/or domestic violence referrals as indicated during the assessment in order to complete the assessment in compliance with HRA policies and procedures. Suggest to the Review Physician referrals to psychiatrist, psychologist, or for Medical Assessment as indicated. Determine a Functional Capacity Outcome and write a supporting justification at the completion of the assessment. Recognize when additional specialty assessments (Psych, Medical, labs) are needed to make a final FCO determination. During re-assessments: Review and re-evaluate clients' progress in relation to measurable goals described in prior Wellness treatment plans, if applicable. Review and re-evaluate client's Functional Capacity Outcome during re-assessment when new medical and/or behavioral health information is provided. Additional duties as needed to ensure compliance with NYC HRA (funder) and UBA's expectations. Qualifications Master’s Degree required NYS professional license LMSW, LCSW or LMHC, Certified Rehabilitation Counselor (CRC), or other credentialed professionals as may be approved by HRA. 1-3 years minimum experience assessing adults with medical and behavioral health issues to determine Functional Capacity Outcome (FCO) or SSA disability Understand how medical/health conditions and associated reported limitations impact on an individual's functional capacity. Understand how behavioral health diagnoses and associated reported limitations impact on individual's functional capacity. Knowledge of what common medications are used to treat. Understand how reported medication side effects can impact on an individual's functional capacity. Requirements to qualify for Federal Disability Benefits as outlined in the SSA Blue Book Listing of Impairments. Ability to administer the SSA Sequential Evaluation Tool. Able to use multiple software systems to review and assess medical and behavioral health conditions as well as document assessment and outcomes in electronic medical records.
